adoptive, foster or step grandparent of the employee; 5) a biological, adoptive, foster or step grandchild of the employee; or 6) a biological, adopted, foster or stepsibling of the employee.

Employees will be notified of available ESSL each time wages are paid.

Notice and Documentation

If the need for leave is foreseeable, employees must provide seven days’ advance notice. Otherwise, employees must generally follow Vital Sign Home Care’s regular reporting procedures for unscheduled absences. When an employee uses sick leave for more than two consecutive shifts, they may be asked to provide documentation of their need for leave.

Payment

ESSL under this policy will be calculated based on the employee's wage rate at the time of absence.

3-6 Bereavement Leave (office-based employees only)

The death of a family member is a time when employees wish to be with their families. If the employee is a full-time Office Staff and loses a close relative, the employee will be allowed paid time off of up to one day to assist in attending to obligations and commitments. For the purposes of this policy, a close relative includes a spouse, domestic/civil union partner, child, parent, sibling, or grandparents. Employees must inform their supervisor prior to taking bereavement leave. In administering this policy, Vital Sign Home Care may require verification of death. Vital Sign Home Care will provide a reasonable amount of break time to accommodate employees desiring to express breast milk. The break time, if possible, must run concurrently with rest and meal periods taken by employees. If the break time cannot run concurrently with rest and meal periods already provided, the break time will be unpaid. The Agency will provide the use of a room or location within close proximity to the employee’s work area other than a bathroom or closet, to express milk in private. This location may be the employee’s private office, if applicable. Employees who wish to request a lactation accommodation may submit a request to the HR Manager. The Agency will respond to this request within five (5) business days and engage in an interactive process with the employee to determine lactation break periods and an appropriate lactation location. If the Agency is unable to provide lactation breaks or a lactation location, or provides a location that does not fully comply with the Ordinance or if the Agency asserts any waiver or variance granted pursuant to the Ordinance, the Agency will, in response to any request for a lactation accommodation, 3-7 Lactation Breaks
